INITIAL INTAKE FORM

Before your appointment

✓ Horse:

Please have your horse ready with a halter and lead in a quiet area. The horse should be dry and clean, including the feet. A flat, firm surface will be required for dynamic assessment. Do not feed your horse 1-2 hours before the visit.

Duration: ~1 hour and 15 minutes

✓ Dog:

Please have your dog ready with a collar and lead in a quiet area of your home away from other dogs and distractions. The dog should be dry, clean, and allowed to a toilet before the session. A flat, firm surface will be required for dynamic assessment. Do not feed your dog 1-2 hours before the visit.

Session overview

✓ Assessment
Following a collection of detailed medical history and establishment of goals, a thorough examination of posture and movement will be carried out (here you will be asked to walk and trot your animal up in a straight line). This is followed by an evaluation of the musculoskeletal system via palpation and joint range of motion, including spinal and neurological examination if required.

✓ Treatment
Each treatment is always individual, based on findings during the assessment and clinical history. The treatment involves a combination of manual techniques and electrotherapies including veterinary laser.
✓ Plan
Depending on animal's needs, therapeutic exercises or simple manual techniques will be prescribed to effectively achieve the set goals. Prescription of a weekly rehabilitation plan is available upon discussion.

To be completed by new clients before the first visit.

Veterinary Referral Form

For new patients, diagnosed conditions and acute cases. Although veterinary referral is not required by Luxembourgish legislation, it is highly recommended as it allows us to focus on the exact diagnosis and provide the best care and outcome possible.
